COMMUNITY LIVING CAMPBELLFORD/BRIGHTON
ACHIEVES CQL PERSON-CENTERED EXCELLENCE ACCREDITATION
CAMPBELLFORD, ONTARIO – Dawn Lee today announced that Community Living Campbellford/Brighton has been awarded the four-year Person-Centered Excellence Accreditation from CQL | The Council on Quality and Leadership. This award confirms that the organization embraces person-centered solutions to improve the quality of life for people receiving supports and services.
At Community Living Campbellford/Brighton it is our mission to support people with an intellectual disability to be able to fully participate in community life, and it is our vision for a community that inspires possibilities to realize the hopes and dreams of all citizens.
Although Accreditation is not yet mandatory in Ontario, Community Living Campbellford/Brighton chooses to have an independent body measure the quality of its support because they want to continually enhance services. The organization has been accredited by CQL since 2000.
When Community Living Campbellford/Brighton partnered with CQL to pursue accreditation, it undertook a rigorous process to review and improve supports and services. The efforts of Community Living Campbellford/Brighton were focused on empowering the people who
receive supports to pursue what really matters in their lives, and achieve their personally defined outcomes.
Community Living Campbellford/Brighton has been awarded CQL accreditation as a result of demonstrating proficiency in the following areas:

CQL Accreditation: Community Living Campbellford/Brighton

Executive Summary

Accreditation Plaque 2015Community Living Campbellford/Brighton began providing supports and services to people with intellectual disabilities since 1960 through the determination of a parent who believed that people with intellectual disabilities belong as members of their community. Currently this organization provides supports and services to approximately 140 people throughout Northumberland County. Community Living Campbellford/Brighton subscribes to an outcome based philosophy that is governed by its Mission, Vision, Principles and Values. Community Living Campbellford/Brighton was first accredited with CQL’s Quality Measures 2005 in October 2006, one of the fist organizations to be accredited using these measures. The Council on Quality and Leadership (CQL) conducted an on-site reaccreditation review March 1-3, 2011 using the Quality Measures 2005.

Community Living Campbellford/Brighton has a very comprehensive plan to monitor Basic Assurances®. The data collection and analysis that the organization completes has led to many examples of identifying trends and focusing efforts to achieve successes. The Health and Prevention Resource Information Binder is an example. It is a great resource and community members are interested in access to a similar resource.

In the area of Community Life® it was determined that the organization has shown meaningful progress in the four years since last having this area measured. During this review, it was determined that all indicators are in the “Effective Results” level. Congratulations to this organization. This is not the first time the organization has been reviewed and determinations have been at this level. Community Living Campbellford/Brighton continues to assess its strengths and successes and develops goals and focuses strategies to continue to move forward in the area of successes and develops goals and focuses strategies to continue to move forward in the area of Conmmunity Life®, recognizing that there is always room for growth.

Community Living Campbellford/Brighton has enhanced its support to people receiving services in the communities and has enhanced its standing in those communities and seeks to move in directions that further enhance communities throughout Northumberland County. CQL commends Community Living Campbellford/Brighton for its strong efforts to bring about positive change in the lives of people supported by the organization, as well as the communities where these services are provided. Community Living Campbellford/Brighton is encouraged to continue on this journey and make new discoveries regarding additional ways to positively impact its diverse community. CQL Accreditation with the Quality Measures 2005 is a four-year term and is awarded to organizations meeting a set of stringent criteria. Because of its willingness for continued learning and quality enhancement, its accomplishments of goals over the past four years and its commitment to strengthening its Community Life® Initiatives, CQL awarded Community Living Campbellford/Brighton another four-year partnership agreement. CQL Accreditation includes an ongoing partnership and periodic on-site visits throughout the four years.

    CQL’s Personal Outcome Measures® are used to identify people’s quality of life outcomes, plan supports, and gather information and data about individual outcomes. CQL facilitators begin the accreditation engagement with Personal Outcome Measures® interviews to demonstrate the linkage between personally defined quality of life and excellence in person-centered services.
     
  • Key Factors and Success Indicators for Organizational Excellence
    CQL’s 8 Key Factors and 34 Success Indicators define quality in terms of person-centered supports and services. These best practices and the resulting quality improvement initiatives are outlined in the The Guide to Person-Centered Excellence.

We invite and encourage active participation by all organizational stakeholders.

  • Bookmark-Driven
    During our onsite consultation, CQL and the organization determine the current status in terms of person-centered services. We bookmark the current situation and provide recommendations for continued improvement and development. Future visits began at this bookmark.

Quality Measures 2005®

CQL’s Quality Measures 2005® is a wide-ranging document with multiple uses in promoting and integrated quality management system. This set of broad-based quality indicators supports organizational quality improvement efforts. Quality Measures 2005® provides a quality design and feedback guide for organizations, support groups and communities. It offers the measures, indicators and measurement methodologies for the CQL Accreditation program.

Third Consecutive Accreditation with Distinction for Community Living Campbellford/Brighton

Executive Summary

CQL Canada conducted an on-site accreditation review at Community Living Campbellford/Brighton from June 13 – 15, 2006 and October 18 – 20, 2006. The five person team from CQL Canada applied the Quality Measures 2005® including the measures of Shared Values, Basic Assurances®, Responsive Services® and Community LifeSM, as well as the Personal Outcome Measures.

During the time at Community Living Campbellford/Brighton it was evident that this organization put the people they support first. Their commitment to an inclusive community was evident in all aspects of service delivery. There were a significant number of outcomes present for people, and the individualized support were in place to maintain those outcomes. The staff was aware where outcomes were not present and the need to develop supports to ensure their attainment.

It was evident during our visits that employees are proud to work at Community Living Campbellford/Brighton, and people supported have a sense of pride as being part of the leadership of this dynamic organization.

Second Consecutive Accreditation with Distinction for Community Living Campbellford/Brighton

Accreditation with DistinctionIn June 2000 Community Living Campbellford/Brighton was the first agency in Ontario to achieve Accreditation with Distinction. The Association has been recognized by Accreditation Ontario for providing supports and services to people with disabilities.

In October 2003 Community Living Campbellford/Brighton became the first association to achieve a 2nd consecutive 3 year Accreditation with Distinction.

The way we do business and support people is guided by the 25 Personal Outcome Measures. Personal Outcome Measures put listening to the person and learning from the person at the center of organization life.

The 25 Personal Outcomes are what people expect from the services and supports they receive from the Association. They refer to the major expectations that people have in their lives. The Personal Outcome Measures are used as a guide to explore with each persons priorities and meaning in each of the 25 areas.

Quality, dedication and the philosophy that “change is the constant” continues to drive Community Living Campbellford/Brighton.
